For Premium Payments 

All premium payments can be made on-line at our Policyholder Service Center. Just log in or register and then click the Pay Bill button on the home page. If you wish to mail your payment, see below for the appropriate address.
 

Most of our customers making a REGULAR INSTALLMENT payment can use the address at right. However, see below for some exceptions.

If you're making a DISABILITY INSURANCE PAYMENT:

You can make a one-time direct debit by following the instructions on our Fax Cover Sheet. 

To avoid future delays (and eliminate installment fees!), you can sign up for ongoing premium debits by completing and returning our Direct Draft Program form.

For Claims
 

If you're a provider
submitting a medical bill:
Use an HCFA1500 or a UB92 and include pertinent Medical Records; make sure the claim number appears on all correspondence.  (To obtain the claim number, call our Customer Service department at 1-800-673-2465, ext. 1300.)  Information on e-billing for California claims is available through Jopari, GUARD’s partner for electronic transaction processing, at 1-866-269-0554.
